Description

Track your allergies by noting down what symptoms you’re experiencing and the severity of the symptoms on a scale of 1-10. For each row you can write down the date, symptoms and then use the checkboxes for how severe your symptoms are.

Great to use to track and log your allergies whether they’re airborne from pollen or scents, food allergies, insect bites or medications.

There’s a blank field in the top right corner where you can add a category, allergy or date range to keep your allergy log organised.

Important information

This is a fillable PDF file which means you can fill out all the form fields in the PDF using Adobe Reader. Please note that you cannot change any of the text or colour that is part of the design.
