Description
Keeping a log of your blood sugar when you have diabetes can be hard, but having a record of these numbers can help you and your doctor get your diabetes under control. This printable comes in two versions; a daily version (with two days per page) and a weekly version.
In the Daily Blood Sugar Tracker, there’s space to write down the date and day.
Fill in what you ate for breakfast, lunch and dinner, and write down your blood sugar levels before each meal, after each meal, and amount of insulin you took. You can also use one of the rows to note down the time you took the reading. (Please ask your doctor for advice regarding the best times for you to check your blood sugar levels. It will depend on your particular health situation)
There’s also space to write down any exercise you did and your readings before and after. Your levels before bed, and there’s also a notes section.
In the Weekly tracker there’s no space to write down the amount of insulin you took, or to describe the exercise you did. It’s a more general overview and the daily version is more in depth. Use the one that suits your situation best. You can use the log to track your blood glucose levels or use it to track the levels for a loved one.

This is a fillable PDF file which means you can fill out all the form fields in the PDF using Adobe Reader. Please note that you cannot change any of the text or colour that is part of the design.
