Description

When you start a new medication it can be helpful to track any side effects. First write down the basic information about your medication and if you’re taking any other medications (in case they interact with each other).

Then write down the date & time, the side effect and the severity of it. This will be helpful for your doctor in adjusting the dose or maybe switching you to a different medication.

It’s great as part of a Medical Binder.
