This medical history tracker lets you keep track of important medical information about your family members. It comes in two layouts, one with checkboxes for various illnesses/disorders and Notes, and one version with lined space to write down Major diseases, Mental Illness, Allergies, Addictions and Notes.

Each page has space for two people, making it ideal for keeping track of parents and grand parents on the same page.

It’s great as part of a Medical Binder or on its own.
